What kind of machine do you use to clean 1" ceramic tiles and grout on a bathroom floor? And when you spray buff a floor, how do you know when it’s time to strip and refinish?

A rotary floor machine with a floor scrub brush will get down into the grout lines. See your Jan-San supplier for suggestions. A revolving brush machine will also work well, if one is available.
Spray buffing will remove finish and after time the floor will have to be refinished. When you no longer see any old finish residue on the buff pad and the gloss is not coming back, it is time to scrub and recoat. You will probably notice the worn areas before that point. Apply finish in the traffic areas and blend in the final coat or two up to the walls. Avoid stripping unless the finish has yellowed (unlikely) or dirt has been sealed in by coating before cleaning.
